THIS NOTICE DESCRIBES HOW YOUR HEALTH AND CONTACT INFORMATION MAY BE OR MAY NOT BE USED OR DISCLOSED BY PREMIEREDOC, INC., HEALTH CARE PROFESSIONALS, AND ASSOCIATES.

1. Introduction

PremiereDoc, Inc., is required by law to maintain the confidentiality of patient health and contact information. PremiereDoc, Inc., will not use or disclose medical information about you without your written authorization, with exceptions stated in the following document.

2. Federal law permits PremiereDoc, Inc., to use or disclose your health and contact information for the following purposes only:

Information will be made available to you upon your request or the request of a parent/guardian.
As and when required by law we may use and disclose your health and contact information to Law Enforcement, Health Oversight Administration, if there are serious threats to your health or safety, if enrolled in the Military or Veteran of armed forces when requested, or if you become an inmate of a correctional facility.
For personal communications − PremiereDoc, Inc., may contact you for appointment reminders, or other information about health related topics you may be interested about.
To health care professionals for verification of appointments, reminders, contact in case of arising issues prior to and following appointments.
Marketing communications only by prior authorization by you to use your health and contact information to send you any marketing materials. We may contact you regarding products or services relating to your care-received, and your health care professionals

3. PremiereDoc, Inc., will not use or disclose your patient health and contact information for the following purposes. PremiereDoc, Inc., will not use or disclose any health or contact information without written authorization of patient or parent/guardian of patient.

PremiereDoc, Inc., will not use or disclose your patient health or contact information for advertising and marketing without your written permission under the consideration of state and local laws.
PremiereDoc, Inc., and associates shall not ask for your health and contact information through phone, electronic or paper mail.

4. Rights to your patient health and contact information:

You have the right to request special restrictions on the use and release of your patient or contact information. PremiereDoc, Inc., is not required to agree to your specific restrictions if it hinders the essential purpose of the service provided by PremiereDoc, Inc., or hampers health care professionals in appointment-making, such as scheduling anonymous appointments with no contact information.
You have the right to view and inspect a copy of your patient health information.
You have the right to request PremiereDoc, Inc., to amend your incorrect or incomplete health and contact information only through the automated database on the website.

5. Personal Information Collected

PremiereDoc Inc. may require personal information when accessing certain pages. All information collected by PremierDoc Inc., including email, usernames, as well as other personal information like gender, insurance, pervious medical history is stored on doctor servers. PremiereDoc Inc. has an SSL Certificate and all personal information is encrypted up to 256 bit. The personal information provided is only available to the patient of whom it belongs to. (See exceptions number 2)

6. Collected Data

PremiereDoc Inc. collects data including IP address and volume of appointments made. No data involving your personal information is collected or sold. All data collected is for marketing purposes solely for PremiereDoc Inc. No information will be provided to third parties or affiliates, with the exception of Federal Law.

7. Public Information

It is the sole responibilty of the user to protect the information they decide to reveal on our discussion boards. PremierDoc Inc. reserves the right to remove information from the forums if it violates the Terms of Service agreement. PremiereDoc Inc. does not provide medical advice, diagnosis, or treatment. All users must abide by these guidelines, any attempt to treat, diagnosis, or offer of advice is the sole responsibility of the individual. PremiereDoc Inc. will remove any and all attempts to provide such information. PremiereDoc Inc. promotes safe internet practices, be careful in disclosing information on our forums.

8. Security

The security of our users is the number one priority here at PremiereDoc Inc. PremiereDoc Inc. uses SSL encryption technology to safe guard against theft. All information provided is encrypted when transferred to and from our servers. PremiereDoc Inc. acts on behalf of our users to ensure security is held at a high standard, however there is no absolutes and therefore we cannot offer a complete guarantee. However if information is obtained that your information is at risk, PremiereDoc Inc. will contact you ( upon the information being presented to us) immediately via email.

9. Healthcare providers and Assistants

All healthcare providers and their assistants are held to their H.I.P.P.A. oath, although this is a third party all information pertaining to patients is held solely by you (healthcare provider). With respect to the advances in technology all personal information must be stored on your servers. Premierdoc Inc. will not store nor hold any personal information provided through the process of treatment, advice and diagnosis.

10. Cookies

PremiereDoc Inc. uses "cookies" to maximize the benefits of this site. In order to use the website you must enable cookies from your pc. PremiereDoc. Inc. does not share any information regarding passwords or other information that cookies store on our site.

11. Changes to Privacy Policy

Any and All changes to this policy will have a two week suspension before they go into effect. Both patients and Doctors will be notified via email if any changes will be made.
